Endometriosis: How to Manage It Every Day?

Endometriosis is a chronic gynecological condition characterized by the presence of endometrial-like tissue outside the uterine cavity. Affecting approximately 10% of women of reproductive age, this condition can present with debilitating symptoms such as chronic pelvic pain, dysmenorrhea (severe menstrual pain), dyspareunia (pain during sexual intercourse), and fertility issues.

Managing endometriosis in daily life requires a multidisciplinary approach that integrates medical treatments, lifestyle changes, and psychological support. As a gynecologist, I would like to outline some evidence-based strategies to help improve the quality of life for patients.

We explore the topic further with Prof. Marco Monti, gynecologist at Arsbiomedica Clinic

Accurate Diagnosis and Personalized Treatment

Endometriosis can only be definitively diagnosed through laparoscopy, a minimally invasive surgical procedure. However, an initial clinical diagnosis based on a detailed medical history, pelvic examination, and imaging (transvaginal ultrasound or MRI) is crucial for initiating treatment.

Therapeutic options include:

  • Analgesic Medications: NSAIDs (Non-Steroidal Anti-Inflammatory Drugs) for pain management.
  • Hormonal Therapies: Combined oral contraceptives, progestins, intrauterine devices (IUD) with levonorgestrel, and GnRH analogs to reduce hormonal stimulation of endometriotic lesions.
  • Laparoscopic Surgery: Recommended for severe cases or when conservative treatments fail. Removal of lesions can improve pain and, sometimes, fertility.

A treatment plan must be personalized, considering the severity of symptoms, the patient’s age, and reproductive desires.

Is There a Cure? Nutritional Approach to Reducing Inflammation

Endometriosis is associated with a state of chronic inflammation. Targeted dietary changes can help reduce symptoms:

  • Favor Foods Rich in Antioxidants: Fresh fruits and vegetables, omega-3 fatty acids (e.g., fatty fish, flax seeds), and whole grains.
  • Limit Dairy, Red Meat, Refined Sugars, and Alcohol: These can exacerbate systemic inflammation.

Although scientific evidence is evolving, a balanced diet, such as the Mediterranean diet, appears to offer significant benefits.

3. Pain Management with Complementary Techniques

In addition to medication, several complementary therapies have proven useful:

  • Pelvic Physiotherapy: Helps reduce muscle tension and improve pelvic mobility.
  • Acupuncture and Massage Therapy: Effective approaches for alleviating chronic pain.
  • Relaxation Techniques: Yoga and meditation can help modulate pain perception and reduce associated stress.

Fertility and Endometriosis

For patients wishing to conceive, close monitoring is essential. Endometriosis can reduce fertility through factors such as altered pelvic anatomy or chronic inflammation. In these cases, it may be necessary to consider options like laparoscopic surgery or assisted reproductive technologies (ART).

Psychological Impact and Social Support

Endometriosis has a significant emotional and psychological impact. Chronic pain and daily limitations can lead to depression and anxiety. Psychological support, whether individual or group-based, is recommended to better manage the condition.

Regular Care and Monitoring

Patients with endometriosis should undergo regular check-ups to assess the progression of the condition and the effectiveness of treatments. Working closely with a trusted gynecologist is crucial for adjusting the treatment as needs evolve over time.

Endometriosis is a complex condition, but integrated management can significantly improve the quality of life for patients. As a gynecologist, I encourage all women not to ignore symptoms and to seek prompt medical help. With a personalized approach, it is possible to live with endometriosis while maintaining a full and fulfilling life.

At Arsbiomedica, through the Arsbiodonna program and with a multidisciplinary team and internal physiotherapy services, we are able to care for the health of women of all ages.
